Please fill out this form on the day and ahead of your visit for us to give you a Clearance Instruction or to provide you the appropriate healthcare you need. Please show us a printed copy or screenshot of your clearance instruction together with a valid ID when you visit us.

By continuing to register, you confirm that you understand that the information provided on the form is true, complete, and correct. Republic Act No. 11332 or the Mandatory Reporting of Notifiable Diseases and Health Events of Public Concern Act requires the full cooperation of all persons to the mandate of the law.
